Amend a Wisconsin Birth Certificate

The state registrar may request the petitioning party to provide the following to their office:

  1. Statement indicating what information on the birth certificate is incorrect and what the correct information should be.
  2. Current certified copy of the birth certificate that includes the incorrect information
  3. Evidence that the current birth record is incorrect. Evidence includes:

The Wisconsin Office of Vital Records reviews the documentation provided by the petitioning party and sends an amendment packet to the petitioning party if a court order is required to amend the birth record.

Upon receiving the amendment packet from the Wisconsin Office of Vital Records, an action can be filed with the Ozaukee County Clerk of Courts upon completing the necessary forms and paying the required filing fee.

  1. Form F-05091– Report of Court Order to Amend a Wisconsin Birth Record
  2. Form CV-472 – Petition to Amend Wisconsin Birth Certificate
  3. Form CV-473 – Order on Petition to Amend Wisconsin Birth Certificate
  4. Current certified copy of the birth certificate
  5. Statement indicating what information on the birth certificate is incorrect and what the correct information should be
  6. Any evidence which verifies the current birth certificate is incorrect

Filing Instructions

Once all the necessary forms are completed, the petitioning party may take them along with the current certified copy of the birth certificate, statement, and evidence to the Ozaukee County Clerk of Courts to have them filed.
